Disclaimer
Ellen Syversen, MPH, NTP, Master Restorative Wellness Practitioner/Functional Nutritionist (both terms used interchangeably):
Before you choose to use the services of a Functional Nutritionist, please read the following information fully and carefully.
GOAL: The basic goal of my work with clients is to encourage people to become knowledgeable about and responsible for their own health, and to bring their health to a personal optimum level. Nutritional therapy is designed to improve your health and wellness in the absence of other non-nutritional complicating factors, and requires a sincere commitment from you, possible lifestyle changes, and a positive attitude.
MY TRAINING: A Restorative Wellness Practitioner/Functional Nutritionist is not trained to provide medical diagnoses, medical codes, or file insurance documentation. No comment or recommendation should be construed as inferring or implying a medical diagnosis. Since every human being is unique, I cannot guarantee any specific result from my programs or promise or guarantee protection from future illness.
Any mention of drugs/medications in the course of consultation is only for the purpose of providing a complete history of drugs/medications that the client is taking and not for Ellen Syversen to judge the appropriateness of the medication. Any change in prescription or dosage is a decision the client makes with his or her physician. If I, the client, suspect I may have an ailment or illness that may require medical attention, then it is my responsibility to consult with a licensed physician immediately. Only a licensed physician can prescribe drugs.
If you are using medications of any kind, you are required to alert Ellen Syversen to such use, as well as to discuss any potential interactions between medications and nutritional products with your pharmacist and prescribing physician.
Rather than dealing with treatment of disease, Ellen Syversen focuses on wellness and prevention of illness through the use of non-toxic, natural nutritional therapies to achieve optimal health.
HEALTH CONCERNS: If you suffer from a medical or pathological condition, you need to consult with an appropriate medical provider. A Restorative Wellness Practitioner is not a substitute for your family physician or other appropriate healthcare provider. A Restorative Wellness Practitioner is not trained nor licensed to diagnose or treat pathological conditions, illnesses, injuries, or disease.
NUTRITIONAL SUPPLEMENTS: If you are under the care of another healthcare provider, it is important that you to inform your other healthcare providers of your use of nutritional supplements. Nutritional therapy may be a beneficial adjunct to more traditional care, and it may also alter your need for medication, so it is important you always keep your physician informed of changes in your nutritional program.
If you have any physical or emotional reaction to nutritional therapy, discontinue use immediately, and contact your Restorative Wellness Practitioner to ascertain if the reaction is adverse or an indication of the natural course of the body’s adjustment to the therapy (a healing reaction).
DISCLAIMER RE BLOODWORK/TESTING: I am a nutritionist, not a doctor, so I can’t diagnose or treat. We are looking at your blood work and test results purely from a functional perspective. We are looking for imbalances before they become pathology. We use an optimal, or functional range, which is the range in which people perform and feel their very best. When you are outside of the optimal range, it means your trending toward imbalance and our work is to restore homeostasis before it becomes a bigger issue. This also holds for any additional testing that we do; food sensitivity testing, stool analysis, adrenal/hormone testing. I am using these tests as additional tools like the food journal, so I have as much information as possible to customize diet and lifestyle recommendations.
Laboratory testing is performed to determine areas of dysfunction and opportunities for healing, not for diagnosis or treatment. Lab testing can help reveal nutritional deficiencies and weaknesses and assist in guiding the process of rebalancing the systems of the body.
COMMUNICATION: Every client is an individual, and it is not possible to determine in advance how your system will react to the supplements you need. It is sometimes necessary to adjust your program as we proceed until your body can begin to properly accept products geared to correct the imbalance. It is your responsibility to do your part by using your nutrition guidelines, exercise your body and mind sufficiently to bring your emotions into a positive balance, eat a proper diet, get plenty of rest, and learn about nutrition. You must stay in contact with Ellen Syversen so she can let you know what is happening and the best course of action.
You should request your other healthcare providers, if any, to feel free to contact Ellen Syversen for answers to any questions they may have regarding nutritional therapy.
Ellen Syversen does not diagnose or treat illness, nor will she be liable for failure to prevent future illness. I understand that Ellen Syversen cannot guarantee results as every client is a unique biochemical individual.
